The North East Local Health Integration Network (NE LHIN) is dedicated to leading a high quality integrated health system for people living in Northeastern Ontario. The NE LHIN plans, funds and works to integrate health services across the region. The NE LHIN is also responsible for the delivery of quality home and community care services for people who call Northeastern Ontario home. The North East LHIN is seeking a dynamic individual to support the work needed to develop a strong relationship with local health service providers, including the negotiation of accountability agreements. Reporting to a North East LHIN Sub-Region Officer, you will be responsible for health service provider liaison, implementation of short and long term strategies by Sub-Region to enhance access to health care services across the continuum of care, and initiatives/strategies to implement the North East LHIN’s Integrated Health Services Plan (IHSP) priorities.
Participate in the development of strategies and opportunities for integration/realignment efforts.
Work collaboratively with other groups and organizations in the sub-region (primary care, physicians, Family Health Team, nurse practitioners, LHINs, diabetes education programs, Community Health Centers and other agencies) to assist with the coordination of chronic disease prevention and management program and service delivery.
Identify gaps and areas of opportunity to improve the delivery of services in the Sub-Region area.
Support performance management and goal achievement of health services within the Sub-Region.
Work across the system with various stakeholders to enhance information management at the practice and program level.
Respond to external information requests as required (e.g. from health service providers, Ministry of Health and Long Term Care).
Post-secondary education in a business administration, health science or equivalent experience working within a health care environment for a minimum of three (3) years.
Experience in negotiating and collaborating with a variety of partners and stakeholders.
Experience working within culturally diverse population groups, including Francophones and Aboriginal/First Nation/Metis.
Excellent oral communications skills to explain complex technical information to senior management, colleagues and stakeholders.
Demonstrated understanding of the Ontario health care system, and the current issues.
Experience in chronic disease management would be an asset but not required.
Experience and training in LEAN methods and project management are considered assets.
Superior oral and written proficiency in English and French is essential.
Salary Range: $71,900 - $89,800
